Question:

How do we code a patient who has a dual chamber ICD in place and comes in for an upgrade to a Bi-V ICD? We would be removing the dual chamber ICD, inserting a Bi-V ICD device along with inserting the LV lead (the RA and RV leads would remain in place from the old ICD and just get hooked up to the new Bi-V ICD deice along with the newly inserted LV lead).
