Question:

Dr. Z, We have an interesting dilemna. One of the "IR" physicians will do angiographies, but then dictate a procedure report with missing information. Example, an aortogram will be done and the MD will include it in the title of the procedure, but will not describe procedure with findings in the body of the report. Subseqently, the radiologist will read the films and dictate another report. He will include the aortogram findings, etc. Is it permissable to use a combination of these dictated reported to assign CPT codes? Thanks.
