Bile Leak Embolization

Question:

"A diagnostic transhepatic cholangiogram was obtained. Following the diagnostic study, a Grebset was used to access a peripheral segment 2 hepatic duct. A 5 French reversed shape catheter was introduced into the common bile duct, and selective catheterization to segment 4 a remnant was successfully performed. As a bile leak site was clearly demonstrated, a 2.8 French microcatheter was introduced into the segment 4 a remnant branch, and embolization was performed with hand-cut Gelfoam particles. The microcatheter was removed, and the 5 French catheter was navigated into the duodenum. After exchanging for a stiff guide wire, a 10 French internal/external biliary drainage catheter was then advanced and secured using sutures. Post placement contrast injection showed good position and drainage, with disappearance of the bile leak. The catheter was attached to gravity drainage."
