Abdominal Muscle wall injection/Anterior Abdominal cutaneous nerve block

Date: Oct 29, 2021

Question:

What are the correct codes for these two abdominal injections?

1. Pain involving skeletal musculature in the right lateral lower abdominal region. It is associated with muscle spasms and enthesopathy involving the right external intercostal muscle, the latissimus dorsi & the serratus anterior. CT was used to advance a needle into the muscles. Bupivacaine was injected - Successful block of the RT inferior external intercostal, serratus anterior and latissimus dorsi muscles. Would this injection be coded as a trigger point with 20553 and 77012?

2. Patient has abdominal pain. It is thought to be related to anterior abdominal cutaneous entrapment syndrome. I was asked to perform an anterior abdominal cutaneous nerve block. The 3 most severe bands of the rectus abdominsis muscles were identified for treatment. A needle was advanced into the medial aspect of rectus abdominis muscle, the linea semilunaris and in the most superficial aspect of the rectus abdominsis muscle and at its margin with the fascia. Kenalog was injected at each level. Would this still be 64999?
