Lt sided pacemaker/lead removal, Implant RT sided pacemaker/new lead

Question:
"Patient developed left-sided breast cancer and is awaiting left-sided mastectomy. Patient presents for left-sided system extraction and implantation of a right-sided device. RV lead was removed, and a new lead was placed. The previously implanted generator was placed on the right side in the new pocket with the new RV lead." Would you recommend billing 33234 and 33216, or would you bill 33234, 33216, and 33222? We are questioning the relocation since the old generator was used, and the work was done to move pockets.
