"Patient came in for redo MVR, TVR, and insertion of Impella LD. A 10 mm Gelweave graft was then anastomosed to the distal ascending aorta using running 5-0 Prolene suture. The graft was brought out the left neck near the sternal notch. The Impella LD was then inserted and positioned in the ascending aorta. The aortic cross clamp was removed. The patient was deaired, rewarmed, and weaned from bypass in the usual manner without difficulty. The Impella was appropriately positioned across the aortic valve with excellent hemodynamics.
