ECMO Daily Management Codes

Question:

I have a question about the ECMO daily management codes (33948 and 33949). First of all, are these codes to be reported by both the facility and the physician? Also, can you tell me what kind of documentation should be included in the chart? Would a critical care or progress note with info on blood flow, oxygenation, etc. be enough, or does there need to be a more formal report?
