Question:

"Indication: known thrombus in left com Iliac, and IVC. Patient arrived and was prepped. US guidance to the right internal jugular vein. IVC placed in the ifrarenal IVC. Attention was turned to the left common femoral vein access using US guidance, noting thrombus in the left common iliac. Using a Penumbra CAT8, attempt to aspirate the thrombus from the common iliac vein and IVC was used with the clot separator. Venogram confirmed additional areas of thrombus resided. Mechanical thrombectomy was performed using AngioJet and Trerotola device. Along with TPA infusion. Followed by venogram showed the thrombus movement into the IVC filter. From the access of the left common femoral, thrombectomy was performed to remove the thrombus from the IVC filter. Then from the right jugular approach, the IVC filter was removed. With follow-up venogram, there was residual thrombus adherent the wall of the IVC. Decision was made to place another IVC filter.
