Closure device failure during EVAR

Question:

How do you normally bill for percutaneous access and closure during an EVAR where the closure device fails and the surgeon dissects down and sutures the artery? Can you still bill code 34713 if it is a large sheath? Code 34812 would not seem right because the cutdown was after the fact and not to deploy the graft. The suture repair seems incidental and not extensive enough to be billable.
