Pacemaker gen change with lead insertion

Question:

We are having a dispute with the cardiologist about how to code this procedure. Can you help? "Patient with a dual chamber pacemaker at end of life. The physician replaces the generator and adds a His bundle lead. A Y-connector was implanted to connect this lead with the old RV lead. The Y-connected lead was inserted into the ventricular port of the new device. The atrial lead was then inserted into the atrial port."
