Question:

Occasionally an EKG will not have the reason as to why it was ordered on the interpretation report or the orders. Is it ok to use the diagnosis/reason that is listed on the progress note/HP note/Consult note of the ordering provider even if the ordering provider is from a different specialty/practice?
