Does this support radiculopathy dx?

Question:

We have several spinal injection cases daily that are lumbar epidurals, and the clinical indication normally will say, "Low back pain extending into the legs." Is this documentation sufficient to use M54.16, which is a covered diagnosis for 62323? Or do we need to have providers specifically add to the reports the term "radiculopathy"?
