Question:
Hello
Dr. Z, What cpt code do you suggest
using for this procedure? Under
ultrasound guidance a 22G needle was percutaneously
advanced into the hypoechoic pancreatic head
mass.Through the needle Dendritic cells which were
previously prepared are injected
into
the pancreatic head tumor. I thought
this should be an unlisted code 48999 with
76942. The doctor suggested
48102.

Z’s Answer:
I
would agree with you to use an
unlisted pancreatic code for this unusual procedure.
This is not a biopsy but a therapy to treat the tumor.
We are not removing any tissue, but instead are
depositing something. A corollary with established codes
and coding guidelines would be ultrasound guided RF
ablation of a liver tumor by placement of a catheter
into a liver tumor and
ablating with a device,.....also, for alcohol ablation
of a renal cyst with CT guidance, we would use the
guidance code and an unlisted renal code
53899. With some of the new
technologies we have to use the unlisted codes until
"New Technology" xxxxT codes are developed.
