Coding
Question: Dr. Z, how would you code
an inferior vena cava filter exchange? Our
radiologists will remove the existing filter and place a
new one. We have been coding these procedures to
37203, 37620 and 36010. Please advise on the correct way
to code this situation. Thanks for your
time.

Answer: Your codes are partly correct.
You are performing two procedures: removal of a filter
and placement of a filter. The removal is reported
as an intravascular foreign body removal which is
reported with codes 37203 and 75961. The new
placement is reported with codes 37620 and 75940.
The catheter placement you're using (36010) is
correct. Remember to report the inferior vena
cavagram (75825-59) if diagnostic imaging is performed
and almost always a diagnostic study would be performed
prior to removal of the filter to be sure there is no
clot in the filter (if there was, the filter would be
left in place).
