Below is one recent coding question (submitted by a ZHealth Online member) & Dr. Zielske's answer--

Coding Question: Dr. Z., I have a case where the patient has a duplicated inferior vena cava.  There are 2 access points-left femoral vein and left internal jugular.  Through both access sites, catheter is placed in the IVC with and IVC-gram.  Then, IVC filters are placed in both.  How would I code this?  I am assuming there are edits on reporting 75825 X2 as well as the filter placement codes. Thanks in advance!  

Dr. Z's Answer:  I would code each diagnostic study, catheter placement and filter placement separately, and the result would be that all four codes would be submitted as duplicates.  You should be paid, but don't be surprised if they ask you for documentation.  This is one of the medical necessity reasons that we perform IVC o grams with filter placements, because if you placed a filter in one of them, large clots from the leg could still get to the lungs and give a pulmonary embolus without this extra protection.
